The effect of platelet-rich plasma (PRP) injection on wound healing and sphincter ani muscle tone in post-repair of grade III-IV perineal tear patients

Abstract

Grade III-IV perineal tears remain a common complication of normal delivery. One alternative therapy for grade III-IV perineal tears is PRP (platelet-rich plasma). Soft tissue healing was faster and better when PRP was applied to the lesion. Growth factors in PRP will stimulate angiogenesis by increasing nutrient supply and therefore shortening healing time. PRP has the potential to reduce the need for repeated treatments. This literature review aims to investigate the effects of PRP injection on wound healing and sphincter ani muscle tone in post-repair of grade III-IV perineal tear patients.This literature review compiling various credible sources from Pubmed, Medline, Science Direct, Google Scholar, etc. and only 34 journals were included.PRP has been widely used in many surgeries and clinical treatments, showing promising results in wound healing, especially in chronic wounds. PRP, showed an increase in new collagen cells and VEGF-A, stimulated proliferation and collagen production, therefore promoting healing. Intramuscular PRP injection in patients with post-repair of Grade III-IV perineal tear resulted in better sphincter ani muscle tone compared to patients post-repair without PRP. This is due to PRP's high growth factor content, which increases growth factor concentration and accelerates the wound healing process and sphincter ani muscle recovery by 2-3 times faster than usual. Platelet Rich Plasma (PRP) administration offers significant benefits in managing perineal tear, providing better wound healing and better anal sphincter muscle tone in grade III-IV perineal tear.
