Profile of chronic rhinosinusitis patients underwent functional endoscopic sinus surgery at Dr. Soetomo Generic Academic Hospital Surabaya

Abstract

Chronic rhinosinusitis is an inflammation of the nasal mucosa and paranasal sinuses that occurs for 12 weeks or longer. Computed tomography (CT) of the paranasal sinuses and nasal endoscopy are important for the diagnosis of rhinosinusitis. These are scored based on the Lund-Mackay and Lund-Kennedy systems. This study aimed to report the profile of chronic rhinosinusitis patients who underwent Functional Endoscopic Sinus Surgery (FESS) at Dr. Soetomo Generic Academic Hospital Surabaya. This descriptive retrospective study used secondary data from medical records. The study population included chronic rhinosinusitis (CRS) patients who underwent FESS at Dr. Soetomo Generic Academic Hospital Surabaya from January 2018 to December 2020. A total of 46 patients were enrolled in this study, with gender ratio of 1:1 (23 male and 23 female patients), with the largest age group was adults (18-59 years) with 43 patients (93.48%). The most common chief complaint was nasal blockage (27 patients, 58.71%). Lund-Kennedy score was less than 2 in 11 patients (23.91%), and score of 2 or more in 35 patients (76.09%). Lund-Mackay score was less than 4 in 12 patients (26.09%), and a score of 4 or more in 34 patients (73.91%). Conservative FESS was performed in 19 patients (41.30%), and radical FESS was performed in 27 patients (58.70%). The characteristics of CRS patients in this study were similar between males and females where the largest age group was adults. The most common complaint was nasal blockage, and the most common risk factor was gastric acid reflux. The most common histopathological result of nasal polyps was mostly inflammatory polyps. Nasal endoscopic examination based on Lund-Kennedy scoring and CT scan examination of the paranasal sinuses based on Lund-Mackay scoring are determinants of conservative or radical FESS.
