N2 - Malocclusion is the third most frequent condition in Indonesia, after periodontal disease and dental caries. The growth and development stage in children is an important stage to receive preventive and interceptive orthodontic treatment during the mixed dentition stage due to the fact that permanent teeth begin to erupt at this time. The aim of this study is to analyze the Index of Complexity, Outcome, and Needs (ICON) and malocclusion prevalence in Kediri, Indonesia. 7- to 13-year-old children. This study was a descriptive observational study with a cross-sectional design that involved 7- to 13-year-old elementary school children at Medowo III Elementary School, Kediri, Indonesia. The study subjects were examined and measured based on the components in the ICON criteria. The prevalence of malocclusion was found to be 50% (22 subjects) in elementary students at III Kediri. From 22 malocclusion subjects, 11 samples (25%) got a score of 5–7 (needs moderate/borderline treatment), and 11 other samples (25%) got a score of 8–10 (really need treatment). Based on the on the ICON examination, 7- to 13-year-old elementary school children at Medowo III Elementary School, Kediri, Indonesia, need orthodontic treatment.
