Abstract

Objective: The purpose of this study was to determine the factors of patients and nurses associated with readmission rates of ACS patient discharge planning at RSUD Dr. Saiful Anwar Malang. Methods: The design of this study was observational analytic with a prospective approach. The study was conducted in August-October 2014 in one of district hospital in Malang. The sample consists of 60 families of ACS patients through a consecutive sampling approach. Data collection using questionnaires and observation sheets. Result: The bivariate analysis showed that there was a relationship between complications of diabetes mellitus (p = 0.030) and complications of heart failure (p = 0.025), history of diabetes mellitus (0.009), patient family participation (p = 0.015), nurse workload (p = 0.001) and patient education (p = 0,000) with the readmission rate of acute coronary syndrome (ACS) patients. The results of multivariate analysis showed that complications of diabetes mellitus (RR 4,383) and complications of heart failure (RR 4,623) and patient education (RR 0.063) affected the readmission rate of acute coronary syndrome (ACS) patients. Conclusion: Factors of medical complications, history of hospitalization, the workload of nurses increased the readmission rate of acute coronary syndrome (ACS) patients. Factors of patient family participation and patient education reduce the readmission rate of acute coronary syndrome (ACS) patients. Factors predicting readmission rates for acute coronary syndrome (ACS) patients are complications of heart failure and diabetes mellitus and patient education.
